Under Forms Dashboard click you will see the bellow forms presented. The 3 Forms that are required to be complete to cross over into ENROLLED status are:

02 - Parent Permission Form.PDF
05 - 2022 NJ Food Form.PDF
06 - Income Verification.PDF

When forms have been completed either on computer, by taking a clear photograph or by scanning you can either e-mail a copy to: registrar@hi4y.org and copy in daycamp@hi4y.org

or upload using the following tabs:
